Application Procedure for a Financial Grant (Meeting Participation Scholarship)

For patients who would like to attend the FSHD Connect Europe Meeting but are unable to cover the costs for registration fee and hotel accommodation themselves, we offer the opportunity to apply for a financial grant (travel expenses are not included). This ensures you can participate free of charge. To participate in the meeting, you must have a good command of English and be able to travel to Amsterdam, the Netherlands.

Scholarship includes:

• Registration fee

• 2 hotel nights stay in one double room (Friday and Saturday)

• 1 welcome Reception

• 1 dinner (Saturday)

• 2 breakfasts (Saturday and Sunday)

• 1 lunch (Saturday)

How does it work?

1. Submit an application: Complete the application form and send it in before March 16.

2. Review process: All applications will be reviewed confidentially.

3. Confirmation: You will receive an email notification if your application is approved before March 25. You will be provided with a code for free regsitration.

4. After allocation:

• Register with the code for free registration before April 8th.

• Book your hotel room before April 8th. You will receive a discount on your bill for 2 nights stay in one double room when you check out of the hotel.

5. Reporting: Scholarship recipients must submit a brief report after the conference, outlining what they learned and how they plan to use the information moving forward.

Important Information:

⦁ The grant (meeting participation scholarship) covers registration fee, hotel (2 nights), and meals (1 dinners, 2 breakfasts, and 1 lunch). Travel expenses are not included.

⦁ Applications must be submitted before March 16.
